A $100 deposit holds the spot and a payment plan is available. The balance is due before testing day. Second and third degree candidates test for $199. Please handle payment with the front desk, because this page does not take card information.

Candidate registration

One form per candidate. If you have two children testing, please fill it out twice. Everything marked with a gold dot is required. If you need to fix something later, fill the form out again with the same email and student name and it will replace your first answer.

01 The candidate Student's full name Which studio does your child train at? Name for the certificate Type it exactly the way it should be printed. This is what gets framed, so middle names and spelling matter. Name to embroider on the belt Usually just a first name. Once it is stitched we cannot change it.

Date of birth Membership type

When did they start training at Warrior? A best guess is fine. We use it on the certificate wall.

04 Uniform and belt size

The uniform and the embroidered belt are both included in the fee, and we order them ahead of camp. Measure by height, not by age.

Uniform size Belt size

Belt size usually matches the uniform size. If you are not sure, pick "not sure" and a coach will measure them at the next class.

05 Camp week safety

We have your child from 9:00 AM to 3:00 PM for four days, plus a 6:00 AM beach morning on August 21. Tell us what we need to know to keep them safe.

Allergies Food, insect stings, medication. Write "none" if there are none. Medications they take or carry Include inhalers and EpiPens, and tell us where they keep them. Anything else about their health Asthma, seizures, recent injuries, anything a coach should watch for. Swimming ability For beach day. Nobody goes past the shoreline without us knowing this. Photo and video release. I give Warrior Martial Arts permission to use photos and video of my child from camp, testing day and celebration day in our marketing and on social media.

06 Why, and your signature Why does your child want to be a Black Belt? We read every one of these out loud on celebration day, so take your time. Where are you with the testing fee? Just so the front desk knows. No card information on this page. I have read the schedule above. I understand that camp on August 17 through 20 and testing day on August 22 are required, and I will have my child there.
